This form is to be used to complete your laboratory assignment under special circumstances when you know that you cannot be present during one of your regularly scheduled weekly laboratory periods and wish to conduct the exercise at an alternate time that same week. 

NOTE:  Lab switch authorizations will NOT be given during the last week of labs (during the LAB PRACTICAL EXAM).

1. Complete the form below; preferably, at least 1 week prior to the time of the exercise to be missed or for valid emergencies.
2. We will respond to your request within 48hrs by email. This response will indicate whether or not your request has been approved. Note, you may need to provide information in addition to that specified in the form below.
3. If your request is approved, the email notification will specify the alternate time and location of the lab that you are authorized to attend for the week requested. Both your regular and substitute Laboratory Instructor will receive copies of the email authorization.
4. Print out your email authorization and TAKE THE FORM WITH YOU.You will NOT be allowed into the laboratory without it. Sign your email authorization and give it to the authorized substitute Laboratory Instructor before the beginning of the laboratory period.
5. The authorized substitute Laboratory Instructor will give you their Entrance Quiz and evaluate Laboratory Performance for that day.  The authorized substitute Laboratory Instructor will provide your regular Laboratory Instructor with these grades, any materials that you submitted, and any other relevant comments.
